The paper Health Spending Account Expense Reimbursement Forms have been discontinued. Expenses must now be submitted through My ASEBP. If you haven't already registered for My ASEBP, you can do so by visiting www.asebp.ab.ca and clicking on "Register for an account" under My ASEBP Login.

The Manulife Plan Member Secure Site allows employees to submit their benefit claims online and access the claim forms. You need to register for access to the Plan Member Secure Site before you can submit your benefit claims online or access the paper claim forms. You can register by clicking on this link (Register) and follow the instructions on the website. Use the "Sign In" "My Group Benefits" at www.manulife.ca to log in if you've already registered.
