Alberta School Employee Benefit Plan (teachers)

The Fort Vermilion School Division covers 100% of benefit premium costs for full-time teachers.  The costs of benefit premiums for part-time teachers are paid on a pro-rated basis according to the percentage of time worked.  Alberta School Employee Benefit Plan is the benefits carrier for teachers.  The current benefits include:

  • Alberta Heath Care
  • ASEBP Plan D, Schedule 2
  • ASEBP Extended Health Care Plan II
  • ASEBP Dental Care Plan III
  • ASEBP Vision Care Plan III

Please use the ASEBP Change Form to update address information, add/delete dependents, change beneficiaries. Ensure your benefit information is up to date at all times!

Recent improvements have been made to the Extended Health Care and Vision Care Explanation of Benefit (EOB) statements that are sent to covered members when a claim is made. These changes came into effect on Monday, October 26, 2009. The information sheet outlines each improvement that has been made to the EOB. Health Spending Account (teachers)

Information about your Health Spending Account (HSA) can be accessed on the ASEBP website: ASEBP Health Spending Account. The HSA Expense Reimbursement Request form as well important information about submitting your request including information on eligible expenses can also be found on the ASEBP website: HSA forms. Your HSA expense reimbursement request can be submitted online using My ASEBP or by paper form. All Health Spending Account payments will be made by direct deposit. You must complete the Automatic Direct Deposit Form and ASEBP must receive it before you will receive any payments from your HSA.

Standard Life (support staff)

The Fort Vermilion School Division covers 100% of benefit premium costs for full-time support staff.  The costs of benefit premiums for part-time support staff are paid on a pro-rated basis according to the percentage of time worked.  Standard Life is the benefits carrier for support staff.

The mandatory Accidental Death & Dismemberment insurance is provided by Citadel Insurance and information can be accessed through the following link: AD&D.

The Voluntary Accident Insurance is also provided by Citadel Insurance and information can be accessed through the following link: Voluntary Accident Insurance.  Employees are responsible for the premiums for this insurance.

Health Spending Account (support staff)

Employment Insurance Benefits

To apply for Employment Insurance (EI) benefits please visit the Service Canada website. Please note that FVSD issues Records of Employment (ROE's) electronically.

Maternity Leave

To apply for Employment Insurance (EI) benefits please visit the Service Canada website. Please note that FVSD issues Records of Employment (ROE's) electronically. Once you are in receipt of benefits please submit the "My Current Claim" page to the Payroll Department for processing of your Maternity SUB plan. To add your new dependent to your benefits complete the ASEBP Change Form (for Teachers) or the Standard Life Change Form (for Support Staff) and submit to the Payroll Department.
