Alberta School Employee Benefit Plan (ASEBP):
The Fort Vermilion School Division covers 100% of benefit premium costs for full-time teachers. The costs of benefit premiums for part-time teachers are paid on a pro-rated basis according to the percentage of time worked. Alberta School Employee Benefit Plan (ASEBP) is the benefits carrier for teachers and can be accessed online at www.asebp.ab.ca. The current benefits include:

The Benefit Handbook (in PDF) can be downloaded at www.asebp.ab.ca under "Quick Links". Common questions can be viewed here: Questions. My ASEBP is online portal that allows covered members to view information related to their ASEBP benefits and general health resources. With My ASEBP you can:
- Request ASEBP ID cards, or print your own
- View, submit edit and delete Health Spending Account (HSA) expenses and download statements
- View health, dental, and vision claims history and transfer unpaid amounts to your HSA
- View your benefit coverage information
- Complete over-age dependant notifications
- Request ASEBP ID cards, or print your own
- Manage your banking information
- Access reliable health information and e-services via Apple-a-Day
Please ensure your benefit information is up to date! Please use the Change Application Form to update your address, add/remove dependents, etc. Please use the Appointment of Beneficiary (Life and AD&D) Form to update or change your beneficiary(ies). Send these forms to the Payroll Department at FVSD Central Office.
Also offered is a Voluntary Accident Insurance Plan: Employee Only Plan or a Family Plan. Details can be viewed here: Voluntary Accident Insurance.pdf and the form for enrolment, cancellation or changes can be accessed here: Form. Employees are responsible for paying the premiums by way of payroll deduction for this insurance, if they choose to participate. Send this form to the Payroll Department at FVSD Central Office.
Health Spending Account (HSA):

The Manulife Plan Member Secure Site gives employees secure and convenient access to their benefits information and offers online claims submission. Employees can log in at any time to electronically submit an expense claim or for details on coverage and claims, including answers to the most pressing questions, including "What can I claim?" and "How much is covered by my plan?".
To access your information or to do an online claims submission login to the Manulife Plan Member Secure Site. Use the "Sign in" as Plan Member at www.manulife.ca to log in if you've already registered or click the 'Register' link if you have not yet registered. Manulife sends information on the process of registration directly to the employee. To contact Manulife for help call 1-800-268-6195. On the Plan Member Secure Site you can:
- Access and update your personal information
- Submit your health, dental and drug claims
- Submit your Health Spending Account claims
- Check the balance of your Health Spending Account
- Access your coverage and personal information
- Consult your employee booklet for detailed descriptions of your benefits
- View the status of your claims
- View or print your benefits card
- Print claim and administration forms if you wish to submit them by mail
- Change or set up direct deposit
The mandatory Accidental Death & Dismemberment insurance is provided by SSQ Financial Group and information can be accessed here: AD&D.

Also available to employees and their immediate family members is an Employee Assistance Program (Resilience) which is a voluntary, confidential, short-term counseling and support service. Click here to view the Resilience brochure. This program provides access to confidential counseling, advice, and coaching to help face life's many challenges, as well as work-life balance services. Help is available anytime by phone, online or in person.
Employment Insurance (EI) Benefits:
To apply for Employment Insurance (EI) benefits please visit the Service Canada website. Please note that FVSD issues Records of Employment (ROE's) electronically.

Once you are in receipt of EI benefits please submit the 'My Current Claim' page to the Payroll Department for processing of your Maternity SUB plan. To add your new dependent to your benefits please complete the ASEBP Change Application Form (for Teachers) or the Request for Change Form (for Support Staff) and submit to the Payroll Department.
